NOTICE OF INTENTION

At least 15 business days before the start of a fund-raising campaign or special event, a "Notice of Intention" [PDF] must be completed, supported by various other documents as specified, and filed with this office. If you have any questions regarding the Notice, call 213-978-1144 and ask to speak with an investigator. After investigation by this office, and "Information Card" [PDF] is issued. The Information Card, which is not endorsement, contains important facts to be considered by a prospective donor in determining the desirability of making a contribution. No solicitation shall be started until the Information Card has been issued.

Note that a separate Notice of Intention must be filed for each fund-raising campaign and/or special event.

Person in Charge of Appeal is the person the Information Card will be mailed to unless otherwise notified.

REPORT OF RESULTS OF ACTIVITY

A Report of Results of Activity [PDF] showing receipts, expenditures, and distribution of net proceeds must be filed within 30 days of the termination of the fund-raising campaign or within 30 days after the special event.

SOLICITOR APPLICATION

A paid solicitor must apply for and obtain a Solicitor's License [PDF] from this office. Each application must be accompanied by the applicant's photograph and fingerprints, and by a fingerprint-processing fee equal in the amount current state of California.

COMMERICAL FUND-RAISERS

An individual other than a bona-fide paid employee of a charitable organization, who receives compensation or consideration to raise funds for a charitable purpose, is required to apply for and obtain a Commercial Fund-Raisers License [PDF] from this office.

BINGO LICENSE APPLICATION

Regular Application [PDF]

An eligible organization must apply for and obtain a bingo license to conduct bingo in the City of Los Angeles. There is an annual fee of $50. The application must be filed no less than 60 days before the date on which the game is to be played or the series of games is to commence.

Senior Application [PDF]

Senior citizen organizations playing bingo for recreational purposes may be granted bingo licenses for periods up to three years.

Supplier Application [PDF]

A bingo supplier must apply for and obtain a license before conducting sales in the City of Los Angeles. There is an annual fee of $100.

INCOME AND EXPENSE STATEMENT

Regular

An Income and Expense Statement must be completed and submitted by the 15th of EVERY month. Charitable Services Section, Los Angeles Police Commission, Commission Investigation Division, upon approval of license will supply the form.

Senior Citizen

An Income and Expense Statement must be completed and submitted by the 15th of EVERY month. The Charitable Services Section, Los Angeles Police Commission, Commission Investigation Division, upon approval of license will supply the form.
