54-Year-Old Critical Missing Elderly Women NR15212mjl

Los Angeles: The Los Angeles Police Department requests the public's assistance in locating Sally Evans. Ms. Evans was last seen on June 16, 2015 in the 5600 block of Vineland Avenue in the City of Los Angeles. Ms. Evans has not been seen or heard from since June 16, 2015 when she left the facility. The personnel at the facility are extremely concerned for her welfare.

Ms. Evans is described as a 54-year-old female white with black hair, brown eyes, 5'02" and weighs approximately 120 pounds. Sally Evans was wearing a pink blouse and pink pants. Sally Evans is diagnosed with Presenile Dementia.
